If you are a Sonoma County senior citizen, or are the caregiver for an aging parent, it is
important for you to know that there are many local resources available to you. At
Green Acres Manor Residential Care Home, we thought it would be helpful to you to

of the following authorities or organizations.
Adult Protective Services (APS)

Our local Adult Protective Services office investigates reports of elder abuse,
exploitation and neglect. The APS also dedicates itself to educating the citizens
of Sonoma County about how to identify and prevent elder abuse. They offer
a 24-hour hotline to answer life-threatening emergencies involving senior citizens.
If you suspect that an elder is being abused, please immediately report it to the
APS at the above phone number.
Area Agency on Aging (AAA)

The AAA is the organization responsible for the creation of the Elder Abuse Council
of Sonoma County. Their focus is on the prevention of elder abuse, in addition
to court advocacy for the victims of elder abuse. The AAA also plans and funds programs
which enable our citizens who are 60 years of age or older to live as indepently
as they can within the community. The AAA Advisory Council of volunteers works in
concert with the AAA, making recommendations to the Board of Supervisors regarding
the needs of our seniors, their caregivers and adults with disabilities.

In Home Support Services (IHSS)

Low income Sonoma County seniors and adults with disabilities who desire to live
safely and independently in their own homes may contact the above phone number
to schedule an appointment with an IHSS social worker. The IHSS provides in-home care
to our low income elders and disabled persons. Once you schedule an appointment,
a social worker will visit you to assess your needs and determine the number of hours
you will be able to hire a caregiver.

This special case management program for frail elderly adults and adults with disabilities,
is designed to assist clients in safe and independent living at home. The program arranges
and coordinates services to help the elderly and disabled persons to manage the tasks
of daily living.

Multipurpose Senior Service Program (MSSP)

MSSP's case management prorgam is available to Medi-Cal eligible, frail elders,
ages 65 and older. The program aims to promote the dignity and independence
of people who wish to avoid being placed in a nursing facility.
Veterans Service Office

If you are a veteran, or the dependent or survivor of veteran and needs assistance
claiming state or federal benefits, please contact the local Veterans Service Office
at the above phone number. The Veteran's Service Office provides advocacy for
obtaining medical services and other services.
